How it work
A comprehensive plan of care is developed based on the patient’s diagnosis, care needs, prognosis, availability of family support, resources in the home environment and community after initial assessment. On-going assessments are done following the frequency of home visits in the plan of care. Plan of cares are updated in consultation with the interdisciplinary team and the patient’s primary care physician.
